New Students

New and Transferring Student Enrollment Information

New and transferring students must enroll in person.  To do so, please download and complete the Student Information Form and bring the following items:
  • Birth Certificate
  • Current Shot Records
  • Proof of Residence in the Derby School District
  • Withdrawal Form from Previous School (If Enrolling After Start of School Year)
  • Completed Student Information Form

* During enrollment please use the Panther Stadium parking lot for easy access to the Media Center


We hope all of you are having an enjoyable, safe summer.  School will be starting in mid August, which means enrollment time is here again. We encourage all parents of pre-enrolled students (students who attended Derby Schools last year) to complete the online information and if needed, update student information.  It is imperative that this information be accurate in the event your child is injured or ill while at school.   If your student is currently attending school with a power of attorney provision or out-of-district waiver, this must be completed each year; new forms may be obtained from the school or board office. All prior school fees must be paid in full before enrolling for the current year.  Curriculum fees, program support fees, technology fees and agenda books are required; yearbook is optional.  At enrollment, you and your child will pick up his/her schedule, agenda and miscellaneous information.  At this time we don’t know if an activity bus will be available; more information will be available closer to the start of the school year.

  • DMS accepts MasterCard and Visa (debit or credit) for payment of fees electronically and in person.  Credit cards are accepted for fees and school lunches; separate checks are required for fees, food service and our parent organization.  Payments made by credit cards will be subject to a transaction fee.

  • FREE/REDUCED LUNCHES & FEES:  If you are requesting free/reduced lunches and fees, enrollment must be completed in person.  Online enrollment will not be available.

  • NEW STUDENT ENROLLMENT:  New students will need to have Birth Certificate, Health Records, Social Security Card, Last Report Card/Withdrawal Card and Proof of Residency; ie; lease/rental agreement, house contract, utility deposit.

PHOTO ID CARDS: All students will be given a temporary ID card at the beginning of school.  Permanent ID cards will be given to the students after school pictures.  If a card is defaced, cracked or lost during the school year, replacement cost is $5.00.  DMS offers lanyards and plastic ID holders the students may purchase for $1.00 each item.

PARENT INVOLVEMENT OPPORTUNITIES AT DMS: Parent involvement proves to be one of the greatest indicators of student success.  There are a number of involvement opportunities available for DMS parents.  PAMS (Partners At Middle School)-PTO, our primary parent involvement group, supports educational activities at Derby Middle School.  A donation for membership is appreciated.  Another opportunity at DMS is the Site Council.  The Site Council members are parents, students, teachers, community members and administrators who meet quarterly to discuss school improvement efforts.  If you would like to be a Site Council member, please indicate so on the parent involvement form.  Site Council meetings are open to all interested parents and community members.  Communities In Schools (CIS) also has opportunities for parental involvement.  Mentoring, tutoring, Watch Dog’s and group facilitation are among the many ways you can be involved.  Please contact DMS to learn more.

PE UNIFORMS: Students enrolled in PE are required to have a PE uniform.  Uniforms for grades 7 - 12 are plain black shorts and a plain gray t-shirt.  PAMS group will offer these at enrollment for $15.00.  Separate checks are required for all spirit wear.  Checks should be made out to PAMS.
